It's one of the most common concerns I hear from clients over 45. And while I wish I could tell you there's a cream that fixes it, that's not reality. But surgery isn't your only option either.
Here's what actually works ... and what doesn't.
What Causes Turkey Neck?
Turkey neck happens due to a combination of factors:
Loss of skin elasticity: As we age, our skin produces less collagen and elastin. The neck skin, which is thin and constantly moving, loses its ability to snap back.
Muscle banding: The platysma muscle runs from your chest to your jaw. Over time, the edges of this muscle can separate and form visible vertical bands.
Fat accumulation or loss: Some people develop fat under the chin (submental fat). Others lose fat, which can make loose skin more apparent.
Gravity: Decades of gravity pulling on facial tissues causes everything to migrate downward.
Sun damage: The neck is often neglected in sun protection routines, leading to accelerated collagen breakdown.
Understanding the cause matters because different causes respond to different treatments.
Can You Actually Fix Turkey Neck Without Surgery?
The honest answer: it depends on severity.
Mild turkey neck (early laxity, just starting to notice): Non-surgical treatments can make a significant, visible difference. You might get 40-60% improvement.
Moderate turkey neck (noticeable sagging, some banding): Non-surgical options can improve it meaningfully, but won't make it disappear. Expect 30-50% improvement.
Severe turkey neck (significant excess skin, prominent bands): Non-surgical treatments will provide some improvement, but surgery will give dramatically better results.
The key is realistic expectations. Non-surgical doesn't mean "as good as surgery." It means meaningful improvement without the downtime, scars, and recovery of surgery.
HIFU for Neck Tightening
HIFU (High-Intensity Focused Ultrasound) is often the most effective non-surgical option for turkey neck because it reaches the deepest layers ... including the SMAS layer that surgeons tighten during a neck lift.
How it works: Focused ultrasound energy heats tissue at specific depths (3.0mm and 4.5mm), triggering collagen contraction and new collagen production.
What to expect:
- Treatment takes 30-45 minutes for the neck
- Mild discomfort during treatment (feels like brief zaps of heat)
- No downtime ... possible mild redness or swelling
- Results develop over 2-3 months as collagen rebuilds
- Results last 12-18 months
Best for: Skin laxity and early jowling. HIFU can also help with mild submental fat.
Limitations: Won't address severe platysma banding (those vertical cords) as effectively as surgery or Botox.

Radiofrequency Skin Tightening
Radiofrequency (RF) uses electrical energy to heat the dermis, stimulating collagen production and causing immediate tissue contraction.
Options include:
- Monopolar RF: Deeper heating, more intense
- Bipolar RF: Shallower, gentler
- RF Microneedling: Combines microneedling with RF for enhanced collagen stimulation
What to expect:
- Treatment series (usually 3-6 sessions)
- Warm, comfortable sensation
- Minimal to no downtime
- Gradual improvement over weeks
Best for: Mild laxity, skin texture improvement, maintaining results from other treatments.
Limitations: Generally less dramatic than HIFU for significant sagging.
Microneedling and Collagen Induction
Microneedling and treatments like Tixel create controlled micro-injuries that trigger your body's healing response, producing new collagen.
For neck specifically:
- Helps with crepey texture and fine lines
- Improves overall skin quality
- Can be combined with PRP or growth factors for enhanced results
Limitations: Won't address deep laxity as effectively as HIFU. Better for surface texture than lifting.

What Doesn't Work for Turkey Neck
Let's save you some money and frustration:
Neck creams: No topical product can tighten loose skin. Some can improve texture or hydration, but none will lift sagging tissue. Don't believe the marketing.
Neck exercises: You can't exercise away loose skin. Strengthening the platysma muscle might even make banding more prominent.
Face yoga: Same issue. Muscle exercises don't address skin laxity.
At-home devices: Consumer-grade RF and microcurrent devices aren't powerful enough to produce meaningful results on neck laxity.
Save your money for treatments that actually work.
When Surgery Is the Better Option
I believe in being honest with clients. Sometimes non-surgical isn't the right answer:
Consider surgery if:
- You have significant excess skin that hangs or folds
- Prominent platysma bands that are very visible
- You want dramatic, long-lasting results
- Non-surgical treatments haven't provided enough improvement
- You're willing to accept 2-4 weeks of recovery
Surgical options include:
- Neck lift: Removes excess skin, tightens muscles
- Platysmaplasty: Specifically addresses muscle banding
- Liposuction: For submental fat (often combined with skin tightening)
A good consultation will tell you honestly which approach makes sense for your specific situation.
