PRP Vampire Facial: What It Actually Does (And What It Doesn't)

The "vampire facial" went viral when Kim Kardashian posted that bloody selfie. The dramatic photos ... face covered in blood ... made it look extreme. But what's actually happening is more interesting than the shock value suggests.
Let me explain what PRP does, how the vampire facial works, and whether it's worth adding to your treatment plan.
What Is PRP (Platelet-Rich Plasma)?
PRP is concentrated healing factors extracted from your own blood.
Your blood contains platelets ... cells that clot blood and release growth factors during wound healing. When we spin your blood in a centrifuge, we separate and concentrate these platelets, creating platelet-rich plasma.
This PRP contains a high concentration of:
- Growth factors that stimulate tissue repair
- Cytokines that reduce inflammation
- Proteins that support collagen production
PRP has been used in medicine for decades ... in orthopedics, wound healing, and surgery. The aesthetic application is relatively newer, but the biology is well-established.
The Vampire Facial: PRP + Microneedling
The "vampire facial" is microneedling combined with PRP. Here's why this combination makes sense:
Microneedling alone: Creates thousands of micro-injuries that trigger collagen production through wound healing.
Microneedling + PRP: The micro-channels allow PRP to penetrate deeper into skin, and the concentrated growth factors amplify the healing response that microneedling initiates.
It's synergistic ... the microneedling creates the opportunity, and PRP provides extra building blocks for repair.
How PRP Enhances Skin Treatments
The process:
- Blood is drawn (like a normal blood test, 1-2 vials)
- Blood is placed in a centrifuge and spun for 10-15 minutes
- This separates red blood cells from plasma
- The plasma layer (golden-yellow, rich in platelets) is extracted
- PRP is applied to skin during or after microneedling
What happens in your skin:
- Growth factors enter through the micro-channels
- They signal your cells to ramp up repair processes
- Fibroblasts are stimulated to produce collagen and elastin
- New tissue forms as the skin heals
Results develop over 2-3 months as new collagen matures.
What to Expect During Treatment
Before:
- Arrive with clean skin, no makeup
- Avoid blood thinners (including ibuprofen) if possible
- No retinoids for a few days prior
During:
- Blood draw: 5 minutes, feels like a normal blood test
- Spinning: 10-15 minutes (you're numbing during this time)
- Microneedling: 20-30 minutes for full face
- PRP application: Applied during or immediately after
- Total time: About 60-90 minutes
The blood on the face: That dramatic look comes from the PRP being spread over the microneedled skin. It's not as bloody as it looks in photos ... the yellow-gold plasma makes a small amount of blood look more dramatic.
After:
- Redness for 24-72 hours (like a sunburn)
- Mild swelling possible
- Skin feels tight as it heals
- No makeup for 24 hours
- Avoid sun, exercise, and heat for 24-48 hours
Realistic Results and Timeline
What PRP vampire facial improves:
- Overall skin quality and glow
- Fine lines and wrinkles
- Skin texture and smoothness
- Acne scars (with repeated treatments)
- Skin firmness and tone
What it doesn't do:
- Lift sagging skin (need HIFU or surgery)
- Remove deep wrinkles (need filler or resurfacing)
- Treat pigmentation (need IPL or other treatments)
Timeline:
- Week 1: Healing, some dryness/peeling
- Week 2-3: Skin looks fresh and glowy
- Month 2-3: Collagen remodeling peaks, best results
- 6-12 months: Results maintained with good skincare
How many treatments: Usually 3-4 treatments spaced 4-6 weeks apart, then maintenance 1-2 times per year.
Who Benefits Most from PRP?
Good candidates:
- People who want to enhance microneedling results
- Those with overall dull, tired skin
- Acne scar patients (as part of a protocol)
- Anyone wanting a natural approach (using your own biology)
- People in their 30s-60s wanting skin quality improvement
May not be ideal for:
- Blood disorders or platelet abnormalities
- Active skin infections
- Certain medications that affect platelets
- People with unrealistic expectations
PRP vs. Other Add-On Treatments
PRP isn't the only thing you can add to microneedling:
PRP: Natural growth factors from your blood. Good for overall rejuvenation and healing enhancement.
PDRN/Salmon DNA: Similar concept, different source. Promotes tissue repair and hydration.
Exosomes: Cell-signaling molecules. Newer technology, may enhance results.
Hyaluronic acid serums: Hydration boost. Less dramatic than PRP but good for moisture.
