Why Your Skin Changes During Perimenopause and What Actually Helps

By Jacqueline Nash, RN

When clients come in and tell me their skincare stopped working or that their skin changed almost overnight, the first thing I ask is where they are in their hormonal journey. More often than not, perimenopause is part of the answer.

Hormonal skin changes are real, they're significant and they're often underexplained. Nobody sits you down and says, "here's what's about to happen to your skin." So you end up trying product after product wondering why nothing is working the way it used to.

Here's what I actually see happening and what I've found that helps.

What Estrogen Loss Does to Your Skin

Estrogen isn't just a reproductive hormone. It plays a direct role in collagen production, skin hydration, wound healing and sebum regulation. When estrogen levels start to decline, usually in your 40s during perimenopause, all of those things are affected.

Collagen production drops. Women can lose up to 30% of their skin's collagen in the first five years after menopause. Skin becomes thinner and less elastic. The bounce-back that skin had in your 30s slows down noticeably.

Oil glands also become less active, so skin that was once normal or combination can suddenly become dry. Moisture retention decreases at the same time, so even if you're applying moisturizer, it doesn't seem to last.

Estrogen also affects how quickly skin repairs itself. Wounds and irritations that used to heal quickly may take longer. Bruising may appear more easily.

Signs Your Skin Is Going Through Hormonal Changes

New dryness or dullness that your usual moisturizer doesn't fix. Skin that feels thinner or more fragile than it used to. Sagging in the lower face and neck that seems to be progressing faster than expected. Adult acne that wasn't there before (this can also be hormonal). Sun spots or hyperpigmentation becoming more pronounced. A general loss of radiance that isn't explained by sleep or stress.

Any of these can be signs that your skin needs a different approach, not just a different product.

Skincare That Actually Helps

Retinol or bakuchiol: Both stimulate cell turnover and collagen production at the surface level. Retinol is more potent but can be irritating on already-sensitized menopausal skin. Bakuchiol is a gentler plant-based alternative with similar effects. Either one used consistently makes a meaningful difference in texture and fine lines.

Hyaluronic acid: Draws moisture into the skin. Especially useful if your skin has become drier during perimenopause. Best applied to damp skin and followed with a moisturizer to seal it in.

Niacinamide: Strengthens the skin barrier, reduces redness and helps with hyperpigmentation. One of the most well-tolerated actives for mature skin.

SPF every day, no exceptions: Existing pigmentation worsens with UV exposure and menopausal skin is more vulnerable to sun damage than it was before. If you're doing any laser or resurfacing treatments, sun protection between sessions is non-negotiable.

These ingredients support your skin, but they can't rebuild collagen or lift tissue the way in-office treatments can. Think of them as maintenance for a foundation that needs reinforcing from the inside out.

Medical Treatments Worth Considering

Collagen induction therapy (microneedling with PRP): Creates controlled micro-injuries that trigger your skin's natural healing response and rebuild collagen. Adding PRP, platelet-rich plasma from your own blood, amplifies the collagen stimulus significantly. This is one of my most recommended treatments for perimenopausal clients because it addresses the underlying collagen loss, not just the surface.

Laser skin tightening and laser sunspot removal: Laser works on two levels: it removes surface damage (sun spots, uneven tone) and it heats the dermis to stimulate new collagen. Fall and winter are the ideal seasons for laser because you're not in the sun between treatments.

Non-surgical facelift (HIFU): For clients dealing with jowls, a softening jawline or neck laxity, this is the treatment that addresses structural change. It works at a deeper tissue level than any topical can reach.

Fountain of Youth mesotherapy: A newer treatment that delivers nutrients, peptides and PRP directly into the skin. Very comfortable. No numbing required. Excellent for improving overall skin quality, hydration and radiance, all things that take a hit during hormonal transition.

HydraFacial: A consistent maintenance treatment that keeps skin clean, exfoliated and hydrated between bigger treatments. Monthly HydraFacials make a real difference in how menopausal skin looks day to day.
