IPL Photofacial: How It Works for Sun Damage, Rosacea & Brown Spots

By Jacqueline Nash, Medical Aesthetics Nurse, Youngstown, Ohio

If you've spent years in the sun and your skin is showing it ... brown spots, redness, uneven tone ... IPL photofacial is one of the most effective treatments to reverse that damage.

It's not a laser. It's not a peel. It's light energy that targets pigment and blood vessels specifically, leaving surrounding skin untouched.

Here's how it actually works.

What Is IPL Photofacial?

IPL stands for Intense Pulsed Light. Unlike lasers (which emit one specific wavelength), IPL emits a broad spectrum of light wavelengths. This allows it to target multiple concerns in a single treatment.

The light energy is absorbed by pigment (melanin) and blood vessels (hemoglobin) in your skin. When those targets absorb the light, they heat up and get damaged ... and your body clears away the damaged cells over the following days and weeks.

The result: brown spots fade, redness calms down, and overall skin tone evens out.

What IPL Treats (And What It Doesn't)

IPL is excellent for:

  • Sun damage and age spots
  • Brown spots and freckles
  • Rosacea and facial redness
  • Broken capillaries and spider veins
  • Uneven skin tone
  • Some types of acne

IPL is NOT ideal for:

  • Melasma (can actually make it worse)
  • Deep wrinkles (need collagen-building treatments)
  • Skin laxity (need tightening treatments like HIFU)
  • Very dark skin tones (higher risk of complications)
  • Active tan or recent sun exposure

Important distinction: Sun spots and melasma look similar but require different treatments. IPL works great on sun spots. On melasma, it can trigger a rebound effect that makes pigmentation worse. We always assess what type of pigmentation you have before recommending IPL.

How IPL Works on Pigment and Redness

For brown spots:

  1. Light energy is absorbed by melanin (pigment) in the spot
  2. The pigment heats up and is damaged
  3. Over 7-14 days, the damaged pigment rises to the surface
  4. Spots darken, then crust slightly, then flake off
  5. New, clear skin is revealed underneath

For redness and rosacea:

  1. Light energy is absorbed by hemoglobin in blood vessels
  2. The vessel walls heat up and collapse
  3. Your body absorbs the damaged vessels over weeks
  4. Redness and visible vessels fade

This is called selective photothermolysis ... the light selectively targets specific colors (brown pigment, red blood) without damaging surrounding tissue.

IPL vs. Laser: What's the Difference?

People often confuse IPL with laser treatments. Here's the distinction:

Laser:

  • Single, precise wavelength of light
  • Very targeted for specific concerns
  • Often more intense, may have more downtime
  • Examples: Pico laser, CO2 laser, Nd:YAG

IPL:

  • Broad spectrum of wavelengths
  • Can treat multiple concerns at once (pigment + redness)
  • Generally gentler with less downtime
  • Good for overall skin rejuvenation

Neither is "better" ... they're different tools for different situations. IPL is often ideal for overall sun damage and redness. Specific concerns (like a single deep spot or tattoo removal) might benefit from laser precision.

What to Expect During Treatment

Before:

  • Avoid sun exposure and tanning for 2-4 weeks
  • Stop retinoids a few days before
  • Arrive with clean skin, no makeup

During:

  • Cool gel is applied to the treatment area
  • You'll wear eye shields to protect your eyes
  • The handpiece delivers light pulses across the skin
  • Sensation: rubber band snaps with brief warmth
  • Full face takes about 20-30 minutes

Immediately after:

  • Skin may be pink and feel warm (like a mild sunburn)
  • Brown spots may look darker initially
  • You can apply makeup and return to activities

Downtime and the Healing Process

IPL has minimal downtime, but there is a visible healing process:

Days 1-3: Mild redness, brown spots look darker

Days 4-7: Darkened spots may develop a coffee-ground texture

Days 7-14: Pigmented spots flake off naturally (don't pick!)

Week 2+: Fresh, clearer skin revealed

Redness from rosacea improves gradually over 2-4 weeks as vessels are absorbed.

Most people need 3-6 sessions spaced 3-4 weeks apart for optimal results. Maintenance treatments 1-2 times per year help prevent new damage from accumulating.

Who Is a Good Candidate for IPL?

Good candidates:

  • Fair to medium skin tones
  • Sun damage, brown spots, or redness
  • No recent tan or sun exposure
  • Not pregnant or breastfeeding
  • Realistic expectations (improvement, not perfection)

May not be ideal:

  • Very dark skin tones (risk of hyperpigmentation)
  • Active tan
  • Melasma (may worsen with IPL)
  • Certain medications that increase light sensitivity
