GLP-1 Weight Loss: How Semaglutide and Tirzepatide Actually Work

You've heard the names: Ozempic, Wegovy, Mounjaro, Zepbound. The GLP-1 medications have changed weight loss more dramatically than anything in decades.
But there's a lot of confusion about how they work, what to expect, and whether they're right for you.
Let me cut through the hype and explain the science.
What Are GLP-1 Medications?
GLP-1 (glucagon-like peptide-1) is a hormone your body naturally produces in response to eating. It signals your brain that you're full and tells your pancreas to release insulin.
GLP-1 medications are synthetic versions of this hormone that last much longer than the natural version. Instead of minutes, they work for days ... which is why most are weekly injections.
Current medications:
- Semaglutide: Ozempic (diabetes), Wegovy (weight loss)
- Tirzepatide: Mounjaro (diabetes), Zepbound (weight loss)
- Others: Liraglutide (Saxenda), dulaglutide (Trulicity)
Semaglutide and tirzepatide are currently the most effective for weight loss.
How They Actually Work in Your Body
GLP-1 medications affect multiple systems:
1. They reduce appetite
GLP-1 receptors in your brain suppress appetite at a neurological level. You genuinely feel less hungry. You're satisfied with smaller portions. The constant background noise of food thoughts quiets down.
2. They slow stomach emptying
Food stays in your stomach longer, so you feel full for extended periods after eating. This is called delayed gastric emptying.
3. They improve insulin response
When you do eat, your body handles blood sugar more efficiently. This reduces the crashes and cravings that drive overeating.
4. They may reduce reward-seeking behavior
Emerging research suggests GLP-1s affect the brain's reward pathways, potentially reducing cravings for alcohol and other compulsive behaviors. The science here is still developing.
The result: eating less feels natural, not like constant willpower.
Semaglutide vs. Tirzepatide: What's the Difference?
Semaglutide (Ozempic, Wegovy):
- Targets GLP-1 receptors only
- Average weight loss: ~15-17% of body weight
- Been available longer, more safety data
- Weekly injection
Tirzepatide (Mounjaro, Zepbound):
- Targets BOTH GLP-1 and GIP receptors ("dual agonist")
- Average weight loss: ~20-25% of body weight
- Newer, potentially more effective
- Weekly injection
Which is better? Tirzepatide shows higher average weight loss in trials. But some people respond better to one versus the other. Your provider can help determine which to try first.
Realistic Weight Loss Results
Clinical trials show impressive numbers:
- Semaglutide: 15-17% average weight loss over 68 weeks
- Tirzepatide: 20-25% average weight loss over 72 weeks
What that looks like:
Starting weight 200 lbs:
Semaglutide: 30-35 lb loss
Tirzepatide: 40-50 lb loss
Starting weight 250 lbs:
Semaglutide: 40-45 lb loss
Tirzepatide: 50-60 lb loss
Important caveats:
- These are averages ... individual results vary widely
- Diet and exercise still matter
- Results require ongoing use
- Weight loss typically plateaus after 12-18 months
Side Effects and How to Manage Them
The most common side effects are GI-related:
- Nausea: Very common early on, usually improves
- Vomiting: Less common, usually dose-related
- Constipation or diarrhea: Common, usually manageable
- Decreased appetite: The intended effect, but can be too strong initially
Management strategies:
- Start at the lowest dose and increase slowly
- Eat smaller, bland meals
- Avoid high-fat and greasy foods
- Stay hydrated
- If nausea is severe, your provider may slow the dose increase
Most side effects improve after 4-8 weeks as your body adjusts.
Serious but rare: Pancreatitis, gallbladder issues, and thyroid concerns have been reported. Medical monitoring is important.
What Happens When You Stop?
This is the question everyone asks ... and the answer isn't what people want to hear:
Most people regain weight after stopping GLP-1 medications.
Studies show that within 1-2 years of stopping, most people regain two-thirds or more of the weight lost. The medications work by changing hormonal signaling, but they don't permanently change your biology.
Options:
- Long-term use (if medically appropriate)
- Lower maintenance doses after reaching goal weight
- Significant lifestyle changes to partially offset regain
- Acceptance that regain is likely and planning accordingly
This doesn't mean the medications aren't worth it ... even temporary improvement in weight has health benefits. But set realistic expectations.
Who Is a Good Candidate?
Good candidates:
- BMI 30+ (obese)
- BMI 27+ with weight-related health conditions
- People who've tried diet and exercise without success
- Those who understand it's a long-term commitment
May not be appropriate for:
- Personal or family history of medullary thyroid cancer
- History of pancreatitis
- Pregnant or planning pregnancy
- People looking for a quick fix they can stop quickly
